If you cannot access the online portal to fill out forms, please arrive 15 minutes early to your appointment to use the tablet in office.

All new patients will receive a prompt to access the patient portal after the first visit is scheduled.

If you need refills on supplements please email or call the clinic directly and we will arrange for a pick-up in our external drop-box.

Dispensary items are not sold to the public and are available only to existing patients of the clinic.
